Oklahoma Residency Documentation
Dove Virtual Academy (DVA) requires proof of residency prior to enrollment and annually as part of the re-registration process. Residency is established through documented evidence provided by the parent or legal guardian. Acceptable forms of Proof of Residency:
__ Real estate deed or mortgage documents
__ Property tax bill
__ Residential lease or rental agreement
__ Water, electric, or gas bill
__ Affidavit of Shared Residence (See next page for Affidavit. It must be signed by both parties and submitted with one of the above residency documents from the person you are residing with.)
*** Affidavit of Shared Residence only needs to be filled out if your family resides with someone else and all bills accepted for Proof of Residence are in their name. The person whose name is on the bill should print/sign their name on the affiant lines.
